Upon initiating direct therapy services, you will be asked to fill out an intake packet.  The intake packet includes:

• New Client Intake Form
• Consent for Services
• Notice of Privacy Practices (HIPAA) and Acknowledgement of Receipt
• Payment Policy Agreement
• Cancellation Policy
• Confidentiality Policy
• Authorization for Medical Release
• Video/photograph Release

Children participating in our Therapeutic Enrichment Group, will receive registration information through their school.
