Upon initiating direct therapy services, you will be asked to fill out an intake packet. The intake packet includes:
- New Client Intake Form
- Consent for Services
- Notice of Privacy Practices (HIPAA) and Acknowledgement of Receipt
- Payment Policy Agreement
- Cancellation Policy
- Confidentiality Policy
- Authorization for Medical Release
- Video/photograph Release
Children participating in our Therapeutic Enrichment Group, will receive registration information through their school.
